Donnett Sinclair, RN, MSN, MBA

Most families are not unprepared because they do not care. They are unprepared because no one has taught them how to talk about what matters before a crisis forces the conversation.

I know this, both professionally and personally.

I am a nurse, healthcare leader, and patient advocate with over 20 years of experience in hospice, home health, and clinical operations leadership. Throughout my career, I have supported many patients and families as they navigated serious illnesses, complex medical decisions, and end-of-life care. Time and again, I observed the same pattern. The greatest pain did not come from death itself; it stemmed from uncertainty, silence, and decisions made under pressure.

That truth became deeply personal when I lost my husband after years of living with End-Stage Renal Disease. Because we had talked early and often, his wishes were known and honored. Even in grief, our family experienced clarity and peace. That experience changed my understanding of what preparation truly means. It is not about giving up hope. It is about protecting the people you love.

I wrote The Last Gift to encourage families to have these conversations with compassion, intention, and care before urgency and fear take control. Through this work, I am creating a movement to make death planning a normal act of love.

This mission also led to the creation of The Last Gift Annual Review™, a straightforward, once-a-year practice that encourages families to openly discuss values, wishes, and decisions while everyone still has a voice. It replaces confusion with clarity and guilt with peace.

Today, I work with families, caregivers, faith communities, and organizations to make these conversations feel normal, compassionate, and accessible. My goal is simple: to ensure families are never left guessing when it matters most.
